You declare that you intend to take a potable water supply connection from the Jorhat Municipal Board (hereinafter referred to as the Board) at the premises stated below for a period not less than 2 (two) years (not applicable for temporary connections) from the date of taking the water supply connection.

You agree to make payment of the processing fees, security deposit and service connection charges at rates as fixed by the Board and as per the terms and conditions of the Board for the water supply connection to your premises.

You agree to pay the water supply consumption bills in accordance with the relevant tariff and also to pay for all such other charges as may become due to the Board at the rates as fixed by the Board from time to time and as per the Board’s terms and conditions, after obtaining the water supplly connection.

You acknowledge that you have read and understood the terms and charges of the board and agree to comply with the same. Additionally, you also undertake to comply with the terms & conditions and the rules (if any) as amended and orders and directions issued from time to time by the Board.

You declare that you (or your landlord, in case you are a tenant) possess a valid holding number under the Jorhat Municipal Board. You further acknowledge that the Board may reject your application in case you (or your landlord, in case you are a tenant) are found not to be possessing a valid Municipal Holding Number.

You acknowledge that all information furnished by you in this application is true and correct to the best of your knowledge and belief, and you bear full responsibility for any false / incorrect statement being furnished & shall face all consequences as per the law.

You acknowledge that the Board reserves the right to withdraw, cancel or extend any water supply connection or and to change on water tariff upon such terms and conditions as it may deem fit from time to time.
